Job summary

CareDiem Home Health is seeking compassionate RNs to provide skilled home health nursing to patients in Rockford and surrounding areas. You will deliver care under physician orders, perform assessments, medication administration, and wound care, and educate families in their homes.

This role requires an Illinois RN license, CPR/BLS, and reliable transportation. Travel to patient homes is required, with flexible FT/PT/PRN scheduling and opportunities for growth.

Qualifications

• Current RN license in Illinois.
• 1+ year home health nursing experience preferred.
• CPR/BLS certification required.
• Reliable transportation and valid driver's license.
• Strong clinical judgment and communication skills.

Responsibilities

• Deliver skilled nursing care under a physician's order.

• Educate patients and families on health conditions and care
• Communicate with physicians and the care team
• Travel to patient homes

What We’re Looking For:

• Current RN license in the state of Illinois
• 1+ year nursing experience (home health preferred)
• CPR certification (BLS)
• Reliable transportation, valid driver’s license, and auto insurance
• Strong clinical judgment, communication, and independence
