Job summary

CareDiem Home Health is seeking compassionate LPNs to provide skilled nursing care in patients' homes in Chicago Heights and surrounding areas.

You will perform assessments, medication administration, wound care, and education for patients and families, while documenting progress in Axxess and collaborating with physicians and the care team. A valid IL LPN license, CPR/BLS, and reliable transportation are required.

Qualifications

• Current LPN license in Illinois.
• 3+ years nursing experience (home health preferred).
• CPR certification (BLS).
• Reliable transportation, valid driver's license, and auto insurance.
• Strong clinical judgment, communication, and independence.

Responsibilities

• Travel to patient homes

What We're Looking For:

• Current LPN license in the state of Illinois

• 3+ year nursing experience (home health preferred)

• CPR certification (BLS)

• Reliable transportation, valid driver's license, and auto insurance

• Strong clinical judgment, communication, and independence
