Job summary

CareDiem Home Health is seeking compassionate LPNs to provide skilled home health nursing to patients in McHenry and surrounding areas.

You will deliver care under physician orders, perform assessments and wound care, document in Axxess, educate families, and travel to patient homes. Reliable transportation required.

Qualifications

• Current LPN license in Illinois.
• 3+ years nursing experience, home health preferred.
• CPR certification (BLS).
• Reliable transportation, valid driver’s license, and auto insurance.
• Strong clinical judgment, communication, and independence.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using our electronic charting system (Axxess).
• Educate patients and families on health conditions and care.
• Communicate with physicians and the care team.
• Travel to patient homes.
