Job summary

CareDiem Home Health is seeking compassionate and reliable RNs to provide skilled home health nursing services to patients in the LaSalle County Area. You’ll deliver skilled nursing care under physician orders, complete assessments, medication administration, wound care, and educate patients and families.

Travel to patient homes is required. The team emphasizes compassionate care, strong clinical judgment, and independence. Illinois RN license, 1+ year experience, and BLS/CPR are required.

Qualifications

• Current RN license in Illinois.
• Minimum 1 year nursing experience (home health preferred).
• CPR certification (BLS) required.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using our electronic charting system (Axxess).
• Educate patients and families on health conditions and care.

What We’re Looking For:

• Current RN license in the state of Illinois


• Current 1+ year nursing experience (home health preferred)


• Current CPR certification (BLS)


• Reliable transportation, valid driver’s license, and auto insurance


• Strong clinical judgment, communication, and independence
