Job summary

CareDiem Home Health seeks compassionate and reliable LPNs to provide skilled nursing services to patients in Kankakee and surrounding areas. You will deliver care under a physician's order, perform assessments, wound care, and medication administration, and document progress using our charting system.

Qualifications

• Current LPN license in Illinois.
• 3+ years nursing experience (home health preferred).
• CPR certification (BLS) required.
• Reliable transportation, valid driver’s license, and auto insurance.
• Strong clinical judgment, communication, and independence.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
