Job summary

CareRing Health in Virginia is seeking a licensed practical nurse to provide patient home health nursing under the supervision of a Registered Nurse. The LPN will perform assessments, administer treatments, and help clients and caregivers manage daily care in the home setting.

The role requires at least one year of nursing experience, current CPR certification, and strong communication, organizational, and independent-working skills.

Qualifications

• Holds a current LPN license in Virginia.
• Minimum of one year of nursing experience.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects with changing priorities.
• Able to employ critical thinking.

Responsibilities

• Accepts assignments of clients from the RN Case Manager.
• Provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatment(s) ordered by the physician and delegated by the RN Case Manager.
• Observes signs and symptoms and reports them to the RN Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data
• Follows Agency policies regarding universal precautions, and safety guidelines
• Fulfills other assignments as requested by the RN Case Manager, VP of Clinical Services, or Director of Professional Services.
