Responsibilities

• Accepts assignments of clients from the R.N. Case Manager
• Provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans
• Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager
• Observes signs and symptoms and reports them to the R.N. Case Manager
• Notes changes in physical or emotional status of clients
• Promotes client and caregiver independence in self-care
• Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data
• Follows Agency policies regarding universal precautions, and safety guidelines
• Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services

Qualifications

• Education and Experience: Holds a current Licensed Practical Nurse license in Virginia
• Has a minimum of one year of nursing experience
• Has current CPR certification
• Has excellent communication, organizational, and human relations skills
• Is able to function with minimal supervision
• Is able to manage multiple projects/changing priorities
• Is able to employ critical thinking skills
