What You Will Do
Accepts assignments of clients from the R.N. Case Manager, provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
Observes signs and symptoms and reports them to the R.N. Case Manager.
Notes changes in physical or emotional status of clients.
Promotes client and caregiver independence in self-care.
Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data.
Follows Agency policies regarding universal precautions, and safety guidelines.
Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
