Job summary

CareRing Health, LLC is seeking a Licensed Practical Nurse in Virginia to provide home-based nursing under the supervision of an RN Case Manager. You will assess, plan, and implement nursing care per physician-approved plans, with emphasis on patient comfort and caregiver education.

Requires an active LPN license in Virginia, at least 1 year of nursing experience, CPR, and strong communication and organizational skills.

Qualifications

• Holds a current LPN license in Virginia.
• Minimum 1 year of nursing experience.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple tasks and changing priorities.
• Strong critical thinking skills.

Responsibilities

• Accepts assignments of clients from the RN Case Manager.
• Provides nursing care through home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatments ordered by the physician and delegated by the RN Case Manager.
• Observes signs and symptoms and reports them to the RN Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care; complies with agency policies.
• Follows agency policies regarding universal precautions and safety guidelines.
• Fulfills other assignments as requested by the RN Case Manager, VP of Clinical Services, or Director of Professional Services.

Responsibilities

What You Will Do

Accepts assignments of clients from the R.N. Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansa. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
