CareRing Health, LLC is seeking a Licensed Practical Nurse to provide skilled nursing services under the supervision of a Registered Nurse in a home care setting.

The role involves carrying out physician-approved care plans, performing treatments, monitoring patient status, and documenting care electronically while ensuring safety and privacy. Strong communication and organizational skills are essential in a collaborative team.

Qualifications

• Holds a current Licensed Practical Nurse license in Virginia.
• Has a minimum of one year of nursing experience.
• Has current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision and manage changing priorities.
• Able to employ critical thinking in patient care.

Responsibilities

• Accepts assignments of clients from the R.N. Case Manager.
• Provides nursing care via home assessment, nursing diagnoses, and physician-approved care plans.
• Performs physician-ordered treatments delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care and follows agency policies.
• Follows universal precautions and safety guidelines.
• Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

Responsibilities

What You Will Do

• Accepts assignments of clients from the R.N. Case Manager
• Provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans
• Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care

Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data

Follows Agency policies regarding universal precautions, and safety guidelines

Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
