Job summary

CareRing Health is seeking an LPN to provide in-home nursing care under the supervision of a Registered Nurse in Virginia. The role includes implementing physician-approved care plans, performing delegated treatments, and monitoring patient status.

You will document care electronically, communicate changes promptly, and collaborate with R.N. Case Managers to ensure high-quality, patient-centered care in the home setting.

Qualifications

• Current Licensed Practical Nurse license in Virginia.
• Minimum of one year of nursing experience.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision and manage multiple priorities.
• Strong critical thinking and problem-solving abilities.

Responsibilities

• Accept assignments of clients from the R.N. Case Manager.
• Provide nursing care per the home assessment, nursing diagnoses, and physician-approved care plans.
• Perform treatments ordered by the physician and delegated by the R.N. Case Manager.
• Observe signs and symptoms and report them to the R.N. Case Manager.
• Note changes in physical or emotional status of clients and update records.
• Promote client and caregiver independence in self-care.
• Utilize electronic documentation and comply with agency policies on timely record submission.
• Follow agency safety guidelines and universal precautions.
• Fulfill other assignments as requested by the R.N. Case Manager or Director of Professional Services.

Responsibilities

What You Will Do

Accepts assignments of clients from the R.N. Case Manager

Provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans

• Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care

Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data

Follows Agency policies regarding universal precautions, and safety guidelines

Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

Education and Experience

• Holds a current Licensed Practical Nurse license in Virginia
• Has a minimum of one year of nursing experience
• Has current CPR certification
