Join our team as a Licensed Practical Nurse Private Duty PRN at Elara Caring.

Job Overview

We are expanding! Many cases available throughout CT with various shifts and flexible hours. Typically caring for one patient at a time, not a whole wing. Be part of a caring, supportive team and make a meaningful difference in families' lives.

Responsibilities

• Provide quality nursing care in patients' homes under RN supervision.
• Develop and report on care plans with interdisciplinary team members.
• Educate patients and caregivers on health matters.
• Accept and document physician orders, obtaining co-signatures as needed.
• Communicate with physicians, patients, families, and staff regarding patient conditions.
• Accurately document all care provided.

Minimum Requirements

• Active Connecticut or compact nurse license.
• High School Diploma or GED.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Optional: 6 months pediatric LPN experience and home health experience preferred.
• Reliable transportation and commitment to quality patient care.
• Physical ability to sit, stand, bend, lift, and move intermittently, with the capability to lift up to 50 lbs.
