Responsibilities

• Provide quality nursing care in patients' homes under RN supervision.
• Develop and follow care plans with interdisciplinary team members.
• Educate patients and caregivers on health management.
• Accept and document physician orders, obtaining co-signatures as needed.
• Communicate with physicians, patients, and families regarding care and concerns.
• Maintain accurate documentation of care provided.

Requirements

• Active Connecticut nurse license or compact license.
• High School Diploma or GED.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Preferred: 6 months pediatric LPN experience and home health experience.
• Reliable transportation and commitment to quality patient care.
• Physical ability to perform job duties, including lifting up to 50 lbs.
