As a Private Duty Nurse/LPN, you will:

• Provide comprehensive nursing care in clients' homes under RN supervision.
• Develop care plans with interdisciplinary team members and report to Field Nursing Supervisors.
• Offer health education to patients and caregivers.
• Accept and document physician orders, obtaining RN co-signatures as needed.
• Follow up with physicians, patients, families, and staff regarding client concerns and condition changes.
• Maintain accurate, timely documentation of care provided.

Requirements

• Active nurse license in Connecticut or a valid compact license.
• High School Diploma or GED.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Minimum 6 months pediatric LPN experience preferred; home health experience is a plus.
• Reliable transportation, dedication to quality care, physical ability to lift up to 50 lbs.

You will report to the Clinical Team Manager.
