Licensed Practical Nurse - Private Duty- Pediatric
Location: New Britain area

As a Private Duty Nurse/LPN, you will provide quality care and contribute to our success by:

  • Providing comprehensive nursing care in the patient's home under the supervision of a registered nurse.
  • Participating in developing the care plan with interdisciplinary team members and reporting to the Field Nursing Supervisors.
  • Providing health education to patients and caregivers.
  • Accepting and documenting verbal and written orders from physicians, with RN co-signature as required.
  • Following up with physicians, patients, families, and staff regarding client issues and reporting client condition changes to physicians.
  • Completing accurate and timely documentation of care provided.

Requirements:

  • Must hold an active nurse license in Connecticut or a compact license.
  • High School Diploma or GED.
  • Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
  • Minimum 6 months pediatric LPN experience preferred.
  • Home health experience is a plus.
  • Reliable transportation.
  • Dedication to quality patient care.
  • Ability to sit, stand, bend, lift, and move intermittently, capable of lifting up to 50 lbs.

You will report to the Clinical Team Manager.
