As a Private Duty Nurse/LPN, you’ll contribute to our success in the following ways:

• Providing quality, comprehensive nursing care in the client’s home under the direction of a registered nurse.
• Participating in the development of the care plan with other interdisciplinary team members and reporting to the Field Nursing Supervisors.
• Providing health education to patients and/or caregivers.
• Accepting and entering verbal and written orders from physicians as required; obtaining co-signature of RN Case Manager on all orders.
• Following up with physicians, clients, family members, staff, and RNs on client problems and concerns. Maintaining communication with physicians including reports of client condition changes.
• Completing accurate and timely documentation of care provided.

What is Required?

• Must have an active nurse license in the state of Connecticut or a compact license.
• High School Diploma or GED
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Minimum 6 months LPN Pediatric experience is preferred.
• LPN Home Health experience is preferred.
• Reliable transportation to perform job duties.
• Dedication to quality patient care.
• Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift up to 50 lbs.

You will report to the Clinical Team Manager.
