Job summary

Ascension is seeking an LPN/LVN to provide comprehensive nursing care in a Saint Thomas Brentwood Primary Care setting in Brentwood, TN. This full-time daytime role emphasizes holistic patient assessments, precise treatments, and patient education to support recovery and ongoing health.

The ideal candidate holds an active Tennessee LPN/LVN license, a BLS certification, and is prepared to work Monday through Friday daytime shifts in a busy outpatient environment.

How you’ll make an impact in this role

• Facilitate holistic patient assessments to identify immediate needs and ensure a personalized, effective care plan is established from the start.


• Deliver precise clinical treatments and medications, vigilantly monitoring for adverse reactions to guarantee patient safety and comfort throughout their stay.


• Perform essential diagnostic lab tests and therapeutic services that provide the clinical team with the critical data needed for timely medical decisions.


• Empower patients and families through education, translating complex treatment plans into clear self-care instructions to ensure a successful recovery after discharge.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.


• Licensed Practical/Vocational Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.

Additional Preferences

• Must be willing to work Monday-Thursday 7am-5:30pm or Monday-Friday 8am-5pm
