Traditional Home Health Care invites a compassionate Licensed Practical Nurse (LPN) to join our pediatric and adult home health team. You will provide one-on-one in-home care, administer treatments, and follow the documented care plan to ensure safety and continuity for patients in their own homes.

Qualifications

• Compassionate LPN to join our pediatric and adult home care team.
• Flexible scheduling options including days, evenings, weekends, per diem.
• Experience delivering high-quality one-on-one home health care is preferred.

Responsibilities

• Provide in-home patient care within assigned cases.
• Implement and document the plan of care for quality and continuity.
• Administer medications, treatments, and therapies as prescribed.
• Assess patient health and coordinate with the Clinical Manager and physician.
• Communicate changes in client condition promptly to the care team.
• Educate and train family members on care routines and safety.

Rate:$28-$32 per hour

Sunbury, PA:
1:Thursday &Friday 6:30pm-6:30am - Saturday &Sunday 10:30pm-6:30am
2.Thursday & Friday 6:30pm-6:30am - Saturday & Sunday 10:30pm-6:30am

Sunbury, PA: Monday - Friday, school accompaniment, 8:00am - 3:00pm/ShikellamySchool District
Sunbury, PA: Trach/Vent overnight shift available
Dewart, PA: Wednesday 7:00am - 6:00pm/Tuesday, Thursday, Friday, Saturday & Sunday 6:00pm - 11:00pm

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Available Nursing Shifts:

• Morning, evening or overnight
• 8’s, 10’s, or 12’s (full shift work)
• PRN, Part-Time, or Full-Time
• No weekend or minimum requirements
