Job summary

Ellen Memorial Rehabilitation and Health Care Center is seeking an LPN to join our care team in Honesdale, PA. The position supports daily living activities, medication administration, and patient-centered care within established policies.

Qualifications

• LPN license is required.
• Responsiveness to patient needs and adherence to care plans.

Responsibilities

• Understand and operate within the LPN scope of practice.
• Maintain resident records and required documentation.
• Evaluate health emergencies and take appropriate steps per regulations.
• Assist with activities of daily living and medication administration as permitted by state rules.
• Follow infection control and safety procedures as trained.
• Attend facility in-services and perform additional duties as assigned.

Responsibilities

• Understand and operate within the LPN Scope of Practice.
• Maintain familiarity with resident records and documents in resident records as required.
• Evaluate health emergencies and determine emergency medical measures to be taken regarding resident care according to state regulations.
• Assist with activities of daily living, environmental orientation, administration of medication, treatments and other care while encouraging self-care and independence, as permitted by state regulations.
• Provide services as indicated on resident care plans.
• Demonstrates knowledge of and follow infection control and safety procedures.
• Attend facility in-services
• Additional duties as assigned.
