Job summary

Ellen Memorial Rehabilitation and Health Care Center is seeking Licensed Practical Nurses (LPNs) for part-time, full-time, and per diem roles at 23 Ellen Memorial Lane, Honesdale, PA. Evening shift 2:45pm - 11:15pm; rate $29.50-$34.40 per hour; $5,000 sign-on bonus.

Qualifications

• LPN license is required.
• Current valid LPN license in PA preferred.
• LPN license verification upon hiring.

Responsibilities

• Understand and operate within the LPN Scope of Practice.
• Maintain familiarity with resident records and documents in resident records as required.
• Evaluate health emergencies and determine emergency medical measures to be taken regarding resident care according to state regulations.
• Assist with activities of daily living, environmental orientation, administration of medication, treatments and other care while encouraging self-care and independence, as permitted by state regulations.
• Provide services as indicated on resident care plans.
• Demonstrates knowledge of and follow infection control and safety procedures.
• Attend facility in-services
• Additional duties as assigned.
