Job summary

Ellen Memorial Rehabilitation and Health Care Center in Honesdale, PA is hiring Licensed Practical Nurses (LPN) for evening shifts (2:45pm–11:15pm). Positions available: part-time, full-time, and per diem.

You will provide medications and everyday resident care, follow care plans, and maintain accurate records while ensuring infection control and safety procedures. A $5,000 sign‑on bonus and comprehensive benefits are offered.

Qualifications

• LPN license required.

Responsibilities

• Understand and operate within the LPN Scope of Practice.
• Maintain familiarity with resident records and documents in resident records as required.
• Evaluate health emergencies and determine emergency medical measures to be taken regarding resident care according to state regulations.
• Assist with activities of daily living, environmental orientation, administration of medication, treatments and other care while encouraging self-care and independence, as permitted by state regulations.
• Provide services as indicated on resident care plans.
• Demonstrates knowledge of and follow infection control and safety procedures.
• Attend facility in-services
• Additional duties as assigned.
