Job summary

Ellen Memorial Rehabilitation and Health Care Center in Honesdale, PA seeks an LPN to provide skilled nursing care in a patient-centered environment. The role supports daily living activities, medication administration, and documentation while adhering to regulatory standards.

Full-time and part-time shifts are available with a $5,000 sign-on bonus and rate negotiable with experience. Weekend schedules may apply.

Qualifications

• LPN license is required.

Responsibilities

• Understand and operate within the LPN Scope of Practice.
• Maintain familiarity with resident records and documents in resident records as required.
• Evaluate health emergencies and determine emergency medical measures to be taken regarding resident care according to state regulations.
• Assist with activities of daily living, environmental orientation, administration of medication, treatments and other care while encouraging self-care and independence, as permitted by state regulations.
• Provide services as indicated on resident care plans.
• Demonstrates knowledge of and follow infection control and safety procedures.
• Attend facility in-services
• Additional duties as assigned.
