Job summary

Elara Caring is seeking an LPN in Home Health to provide skilled nursing care in patients' homes under the supervision of an RN and physician. You will follow the Plan of Care, monitor conditions, educate patients and caregivers, and collaborate with the care team. Travel locally (about 50%) is required.

• Must hold an active LPN/LVN license in your state.
• Associate Degree in Nursing (ADN) from an accredited program.
• Minimum 1 year of clinical nursing experience.
• Home health or hospice experience preferred.
• Strong clinical assessment and communication skills.
• Ability to lift up to 50 pounds and to travel locally (approx. 50%).

Responsibilities

• Providing skilled nursing care under the direction of an RN and physician.
• Following the Plan of Care and physician orders during visits.
• Monitoring patient conditions and reporting changes.
• Educating patients and caregivers on disease management, medications, and self-care.
• Supporting patient safety through individualized care planning.
• Documenting assessments, interventions, and updates accurately.
• Collaborating with physicians, RNs, and the interdisciplinary team.
• Identifying opportunities to improve patient care and updates to the Plan of Care.
• Helping reduce avoidable hospitalizations through proactive management.
• Delivering compassionate, respectful, professional care.

What You'll Do

• Providing skilled nursing care under the direction of an RN and physician
• Following the established Plan of Care and physician orders during each patient visit
• Monitoring patient conditions and reporting significant changes to the RN Case Manager and care team
• Educating patients and caregivers on disease management, medications, and self-care techniques
• Supporting patient safety through individualized care planning and risk prevention strategies
• Documenting patient assessments, interventions, and medication updates accurately and timely
• Collaborating closely with physicians, RNs, and interdisciplinary team members to ensure quality outcomes
• Identifying opportunities to improve patient care and recommending updates to the Plan of Care
• Helping reduce avoidable hospitalizations and emergency room visits through proactive patient management
• Delivering compassionate, respectful, and professional care to patients and families

What You'll Bring

• Graduate of an accredited Associate Degree Nursing program
• Current state LVN/LPN license in good standing
• Minimum of one (1) year of clinical nursing experience
• Home Health or Hospice experience preferred
• Strong clinical assessment and communication skills
• Ability to lift, push, pull, and support up to 50 pounds as needed
• Valid driver's license, reliable transportation, and current auto insurance
• Willingness to travel locally (approximately 50%)
