Job summary

Elara Caring is offering a home health LVN/LPN position in Lawton, OK, working with a care team to provide Right Care at the Right Time in patients’ homes.

Responsibilities include following care plans, monitoring progress, and educating patients and families. Requires LVN/LPN license, 1 year clinical experience, and ability to lift up to 50 pounds with reliable transportation for travel to patient homes. Hospice/home health experience preferred.

Qualifications

• Must hold a State LVN/LPN license and graduate from an accredited nursing program.
• 1 year of clinical care experience required.
• Must be able to lift up to 50 pounds and assist with patient positioning and transfers.
• Must have a valid driver’s license or reliable transportation for travel between patient homes.

Job Description:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
