Job summary

Elara in Michigan is seeking a Licensed Practical Nurse (LPN) to join our home health team serving Newaygo, Mecosta, Mason and Oceana counties. The role focuses on compassionate, patient-centered care delivered in patients’ homes.

You will follow individualized care plans, document care, and collaborate with RNs and physicians to support positive outcomes while traveling between patient homes. An active LPN license and dependable transportation are essential.

Qualifications

• State-licensed LPN in Michigan.
• Graduate of an accredited nursing program (certificate/diploma/associate).
• 1 year of clinical care experience preferred.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting preferred
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
