Job summary

Elara Caring is seeking a Licensed Practical Nurse to join our home health team serving Van Buren County and Northern Berrien County in Michigan. The role runs Monday to Friday, 8:00am–5:00pm, Full Time, with responsibilities across direct patient care and coordination with the RN Case Manager.

You will follow individualized care plans, monitor patient progress, educate patients and families, document medications, and communicate with physicians to ensure safe, coordinated care.

Qualifications

• Current State License as an LPN
• Graduate of an accredited Nursing program (Certificate/Diploma/Associate)
• At least 1 year of clinical care experience
• Ability to lift up to 50 pounds and transfer patients
• Valid driver's license or reliable transportation for travel between patient homes

Responsibilities

• Follow patient care plans and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Collaborate with RN Case Manager to update care plans as needs change.
• Document medication regimens and update medication profiles.

Primary Coverage: Van Buren County and Northern Berrien County

Schedule: Monday-Friday 8:00am-5:00pm, Full Time

As a Licensed Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.

What is Required?

• Current State License as a Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
