Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to deliver patient-centered care in home health. You will follow individualized care plans, monitor progress, and educate patients and families while coordinating with the care team to ensure safety and quality outcomes.

Applicants should have a valid LVN/LPN license, nursing education, and at least 1 year of clinical experience, with the flexibility to travel between patient homes.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited nursing certificate, diploma, or associate program.
• 1 year of experience in a clinical care setting.
• Ability to lift up to 50 pounds and move patients/equipment.
• Valid driver’s license or reliable transportation to travel between patient homes.

Job Description

As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
