Job summary

Elara Caring is seeking a Licensed Practical Nurse to contribute to high-quality home health care in the Mentor, OH area. You will follow individualized care plans, monitor patient progress and support families with self-care techniques, coordinating with the RN Case Manager to adapt plans as needed.

Ideal candidates have a valid LPN/LVN license, at least 1 year of clinical care experience, and the ability to lift up to 50 pounds. Hospice/home health experience is preferred.

Job Description

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.
