Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to join our home health team in Louisiana. You will follow individualized care plans, monitor patients, and collaborate with RNs to adjust plans as needs evolve.

The role emphasizes compassionate, high-quality care delivered in patients’ homes. Requirements include an active LVN/LPN license, at least 1 year of clinical experience, ability to lift up to 50 pounds, and reliable transportation to travel between patient homes.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse (LVN/LPN)
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and locations
• Hospice or home health experience is preferred

What is Required?

• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
