Job summary

Elara Caring in Michigan seeks a dedicated Licensed Practical Nurse to deliver compassionate home health care across Newaygo, Mecosta, Mason and Oceana counties. You will follow individualized care plans, coordinate with RNs, document medications, educate patients and families, and help prevent unnecessary hospital visits while ensuring safety and quality care in patients' homes.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting preferred.
• Must possess a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as patient needs change.
• Document medication regimens and update medication profiles.

As a Licensed Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting preferred
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
