Job summary

Elara Caring in Lorain, OH is hiring a Licensed Practical Nurse to join our home-based care team, delivering the Right Care at the Right Time in patients' homes. You will work with the RN Case Manager to adapt care plans and provide essential patient education.

You will monitor patient progress, document medications, and communicate with physicians, patients, families, and the care team to ensure coordinated care and reduce hospitalizations.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Graduate of an accredited diploma/associate nursing program.
• Minimum 1 year of clinical care experience.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license or reliable transportation for travel between patient homes.

Responsibilities

• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
