Job summary

Elara Caring in Owosso, MI seeks a Licensed Practical Nurse to provide high-quality home health care following patient-specific plans and physician orders. You will monitor progress, educate families, and coordinate with the care team to ensure safety and continuity of care.

The role requires 1 year of clinical experience, a valid LPN license, and the ability to lift up to 50 pounds. Reliable transportation for travel between patient homes is needed.

Qualifications

• Must hold a valid State LPN license.
• Graduate of an accredited nursing program (Certificate/Diploma/Associate).
• Minimum 1 year of clinical care experience.
• Ability to frequently lift, push, pull, and transfer up to 50 pounds.
• Valid driver’s license or reliable transportation for travel to patient homes.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.

• Document medication regimens and update medication profiles.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
