Job summary

Elara Caring is seeking a Licensed Practical Nurse to join our team in Rockford, IL and surrounding areas, delivering trusted home healthcare. The role focuses on following care plans, monitoring patients, and collaborating with the care team to ensure safe, effective treatment.

Applicants should have an active LPN license and at least 1 year of clinical care experience. A valid driver's license or reliable transportation for visiting patient homes is required.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Coordinate with the RN Case Manager to update care plans as needs change.
• Document medication regimens and update profiles per guidelines.
• Communicate with physicians, patients, families, and the care team for coordinated care.
• Help prevent avoidable hospitalizations through proactive care.
• Deliver compassionate, high-quality care for positive patient outcomes.

This nurse will support Rockford, IL, and the surrounding areas.

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred

The base salary for this position is $32 to $35 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
