Job summary

Lake Cumberland Regional Hospital, part of Lifepoint Health, is seeking an LPN for our Family Practice Clinic in Somerset, KY. This full-time role operates 8:00am–5:00pm, Monday through Thursday, supporting ongoing patient care in a community-focused setting.

Under supervision of an RN, you will collect data, assist with plans of care, and handle essential nursing tasks including wound care, medications, and patient education. Kentucky licensure and BCLS are required.

Qualifications

• Graduate of an approved practical nursing program.
• Current LPN/PN license to practice in Kentucky.
• BCLS certification within 30 days of hire.

Responsibilities

• Under RN supervision, assists in patient assessment and data collection.
• Performs plan of care interventions including wound care and specimen collection.
• May administer medications as directed by the RN/physician.
• Documents patient care provided and communicates status to the care team.
• Assists with activities of daily living and maintaining patient comfort.
• Schedules procedures and coordinates patient flow as ordered.

How you'll contribute

• Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care.
• May administer medication.
• Documents patient care given.
• Assists RN with development of plan of care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• Works as an advocate for the physical and emotional well-being of the patient.
• Schedules, appointments and procedures as ordered by the provider.
• Obtains insurance re-authorizations as requested for testing and scheduled procedures.
• Attend clinic meetings.
• Maintains a clean and safe working environment.
• Reports all incidents, hazards, drug interactions, and initiate appropriate action.
• Completes safety instructional programs.
• Assist other team members when time permits.

What we are looking for:

• Education: Graduate of an approved practical nursing program

License:

• Current LPN/PN to practice in the Commonwealth of Kentucky and be in good standing with the Board of health Professions
• Active Kentucky license (single state or compact) required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.

Certifications:

• BCLS within 30 days of hire

Minimum Work Experience:

• Knowledge of ICD-9, ICD-10, CPT codes, insurance benefits and healthcare backgrounds
