Job summary

CareDiem Home Health is seeking compassionate and reliable RNs to provide skilled home health nursing to patients in the Bureau County Area. Deliver skilled nursing care under physician orders and travel to patient homes to assess and care for clients.

The role requires an Illinois RN license, 1+ year nursing experience (home health preferred), CPR/BLS, reliable transportation, and strong clinical judgment with excellent communication.

Qualifications

• Current RN license in Illinois
• 1+ year nursing experience (home health preferred)
• CPR certification (BLS)
• Reliable transportation, valid driver’s license, and auto insurance
• Strong clinical judgment, communication, and independence

Responsibilities

• Deliver skilled nursing care under a physician’s order
• Complete assessments, medication administration, and wound care
• Monitor and document patient progress using our electronic charting system (Axxess)
• Educate patients and families on health conditions and care
