Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset in Patchogue, NY is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in our SNF and Long Term Care Units. You will administer medications, monitor vital signs, and educate residents and families.

We offer a $3,000 sign-on bonus, daily pay options, and a comprehensive benefits package. Current LPN licensure is required for this on-site position.

Qualifications

• Current LPN license in good standing.
• Experience in long-term care or skilled nursing preferred.
• Ability to monitor and report resident conditions and outcomes.

Responsibilities

• Provide nursing care to SNF and long-term residents.
• Administer medications, including IVs.
• Take and record vital signs and monitor patient outcomes.
• Educate residents and families on care plans and treatments.
• Coordinate with physicians, NPs, and care team; supervise CNAs.
