Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing and long‑term care teams. The position involves medication administration, including IVs, vital signs, and direct resident care with outcome monitoring.

Responsibilities include educating residents and families, coordinating with physicians and the care team, and supervising CNAs to maintain high standards of care.

Qualifications

• Current LPN license is required.
• Experience in skilled nursing or long-term care settings is preferred.
• Ability to perform medication administration and IVs.
