Job summary

St. Joseph Hospice in Sibley, LA is hiring Part-Time and PRN Licensed Practical Nurses (LPN) to join our hospice team. You will provide nursing and supportive care under the supervision of an RN, following the Interdisciplinary Plan of Care.

Requirements include a Louisiana LPN license, CPR certification, 2–3 years of LPN experience, and reliable transportation. Hospice experience is preferred. We offer ESOP benefits, competitive pay, mileage reimbursement, and opportunities for career

Qualifications

• Current Louisiana LPN license is required.
• Minimum of 2-3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Coordinates care with attending physicians and hospice team to optimize patient care and resources.
• Documents all services and submits them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Reports changes in patient condition to the RN.
• Exhibits strong time management skills.

Job Duties
