Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to join our Skilled Unit and Long Term Care Unit. You will provide medication administration, take vital signs, and deliver comprehensive resident care.

We offer a $3,000 sign-on bonus, daily pay options, and a benefits package including medical, dental, vision, and union benefits. Current LPN license required, with opportunities for advancement and free uniforms.

Qualifications

• Current LPN license in good standing.
• Experience in SNF/Long Term Care preferred.
• Ability to supervise CNAs and coordinate with care team.

Responsibilities

• Provide care for residents in Skilled and Long Term Care units.
• Administer medications, including IVs.
• Take and record vital signs and monitor outcomes.
• Educate residents and families on care plans and treatments.
• Communicate with physicians, NPs, and family members.
