Job summary

Qualifications

• Current LPN license in Kentucky.

Responsibilities

• Provide bedside care to residents including personal hygiene.
• Record vital signs (BP, pulse, resp) and document accurately.
• Collect health information and relay to RNs/physicians.
• Administer oral meds and injections as prescribed.
• Test blood sugar levels and manage basic treatments.
• Monitor changes in residents and report to nursing team.

LPN Responsibilities:

• Provide bedside care to patients, including personal hygiene maintenance
• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)
• Collect the patient’s health information
• Relay information between patients, registered nurses, and physicians
• Give oral medications and injections to patients
• Test a patient’s blood sugar levels
• Urinary catheter insertion and care
• Assist patients with personal hygiene
• Monitor for changes in patients
• Proper charting, documentation, and maintenance of patients’ medical recordsAsks questions when in doubt for clarification by the Registered Nurse.
• Acts as a role model for ancillary personnel.
• Maintains the confidentiality of the patient’s clinical records.

LPN Requirements:

• Current LPN license in the state
• At least one year of LPN experience in a long-term Care Setting is preferred.
