Job summary

Creekwood Nursing and Rehabilitation in Russellville, KY seeks an LPN or RN Unit Manager to lead the clinical team and oversee resident care on the unit.

Responsibilities include supervising RNs, LPNs, and CNAs, conducting assessments, updating care plans, and ensuring regulatory compliance with physician orders and facility guidelines.

Qualifications

• Current LPN or RN license in the State.
• At least one year of experience as a LPN or RN in a Long Term Care setting preferred.

Responsibilities

• Delivers basic nursing care to residents as ordered by a physician.
• Directs RNs, LPNs and CNAs on the unit to ensure high quality care.
• Conducts ongoing resident assessments and revises care plans as needed.
• Collaborates with the ADON and DON to resolve care-related issues.
• Documents interventions and resident education timely and accurately.
• Conducts rounds to assess conditions and practices.
• Documents nursing notes in charts promptly.
• Performs medication administration and IV duties as ordered.
• Monitors resident progress and suggests treatment plan improvements.
• Observes resident status and reports findings to supervisors.

LPN/RN Unit Manager Job Duties:

• Monitors facility clinical systems to effectively facilitate quality resident care in accordance with laws, regulations and facility guidelines.

• Successfully delivers basic nursing care to residents under in accordance with care plans as ordered by a physician.

• Directs and develops RNs, LPNs and CNAs working on the unit to ensure the delivery of high quality care and services in accordance with all laws, regulations and guidelines.

• Conducts on-going assessments of residents to identify care needs and makes needed revisions to the care plan based on changes in resident condition.

• Works in conjunction with the ADON and DON to provide resolution of grievances or concerns related to resident care or family issues.

• Documents interventions and resident or family education efforts timely and accurately.

• Conducts rounds within the nursing department to assess resident conditions and clinical practices.

• Accurately documents nursing care notes in resident charts in a timely manner.

• Performs medication administration and IV duties in accordance with the care plan as ordered by a physician.

• Monitors progress of the residents and makes suggestions regarding treatment plans which may further meet their care needs.

• Observes resident behavior, vital signs, symptoms, responses to treatment and reports findings to nursing supervisors.

Unit Manager Requirements:

• Current LPN or RN license in State.

• At least one year of experience as a LPN or RN in a Long Term Care setting preferred.
