Qualifications

• Current Tennessee LPN license.
• Experience providing care in SNF or Long Term Care settings preferred.
• Day shift and overnight shift available (12-hour shifts).
• Ability to collaborate with physicians, NPs, families and other health professionals.

Responsibilities

• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with outcome monitoring.
• Educate residents and families about care plans and procedures.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs as assigned.
