Job summary

Gallatin Center for Rehabilitation and Healing is hiring a Licensed Practical Nurse (LPN/LVN) for day or night shifts. The role includes SNF and long-term care duties, with a focus on medication administration, vital signs, and compassionate resident care.

Responsibilities

• Provide care for residents in SNF and LTAC units.
• Medication administration, including IVs.
• Record vital signs and assess patient status.
• Provide resident and family education and support.
• Communicate with physicians, NPs and families; coordinate with care team.
• Supervise CNAs and assist with care planning.
