Job summary

St. Joseph Hospice in Richland, MS is seeking a Full-Time Licensed Practical Nurse (LPN) to provide compassionate care under the direction of the RN, following the Interdisciplinary Plan of Care.

In addition to clinical responsibilities, the role involves coordinating with the hospice team and documenting all services. ESOP ownership, competitive pay, generous PTO, disability coverage, and mileage reimbursement are included.

Qualifications

• Current Mississippi LPN license is required.
• Minimum 2–3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver's license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care under the RN.
• Coordinate with attending physicians and hospice team to optimize patient care.
• Document all services on appropriate forms.
• Demonstrate knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition to the RN.

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
