Job summary

Pennypack Nursing and Rehabilitation Center in Philadelphia is seeking Licensed Practical Nurses (LPNs) to join our nursing team on part-time, per diem, and full-time roles across all shifts. The LPN will deliver hands-on resident care, administer medications, and collaborate with the interdisciplinary team to support positive outcomes in a skilled nursing and rehabilitation setting.

Candidates must hold an active LPN license and complete an accredited practical nursing program; prior long-term

Qualifications

• Active LPN license in the applicable state.
• Completion of an accredited practical nursing program.
• Prior experience in long‑term care, rehabilitation, or skilled nursing preferred.
• Strong clinical knowledge, communication, and organizational abilities.
• Ability to work both independently and collaboratively.
• Dedication to delivering resident‑centered care.

Responsibilities

• Deliver direct nursing care following physician orders and individualized care plans.
• Administer medications and treatments accurately and safely.
• Observe residents’ conditions and promptly report any changes to the RN or physician.
• Complete timely and accurate documentation in the electronic medical record.
• Support admissions, discharges, and resident transfers.
• Provide guidance and oversight to CNAs and assist with daily care needs.
• Adhere to infection control, safety, and regulatory compliance standards.
• Communicate effectively with residents, families, and members of the healthcare team.

1 PT- 7am to 3pm

1 PT- 3pm to 11pm

Pennypack Nursing and Rehabilitation Center is looking for dedicated and compassionate Licensed Practical Nurses (LPNs) to join our nursing team. We are currently hiring for full-time, part-time, and per diem roles across all shifts. In this position, the LPN will deliver hands-on resident care, assist with clinical operations, and collaborate with the interdisciplinary team to support positive outcomes in a skilled nursing and rehabilitation setting.

Licensed Practical Nurses Responsibilities
