Job summary

Silver Stream Rehabilitation and Nursing Center is seeking a compassionate Licensed Practical Nurse (LPN) for Day Shift. This 3-day workweek schedule features three 12-hour day shifts (7:00 AM–7:00 PM) with every other weekend, offering stable hours and work-life balance.

The LPN will administer medications, monitor residents, document care, supervise CNAs, and collaborate with the interdisciplinary team while ensuring compliance with state and facility policies.

Qualifications

• Current PA LPN license required.
• Long-term care experience preferred.
• Strong clinical, communication, and organizational skills; ability to work effectively in a team.

Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. This position offers a consistent schedule of three 12-hour day shifts per week (7:00 AM-7:00 PM), including every other weekend, providing an excellent work-life balance while making a meaningful difference in the lives of our residents.

Responsibilities:

• Administer medications and treatments as prescribed

• Monitor residents' conditions and report changes to the RN Supervisor and physicians

• Provide direct nursing care in accordance with facility policies and standards of practice

• Accurately document resident care and services provided

• Supervise and support Certified Nursing Assistants (CNAs)

• Participate in care planning and interdisciplinary team meetings

• Ensure compliance with all state, federal, and facility regulations

Qualifications:

• Current LPN license in the Commonwealth of Pennsylvania
