Job summary

Shippenville Nursing and Rehab is seeking a compassionate Licensed Practical Nurse (LPN) to provide direct patient care on a per diem basis in our long-term care facility. You will administer medications, monitor residents, and collaborate with the care team to deliver personalized support.

Qualifications

• Current LPN license in Pennsylvania.
• Ability to work in a fast-paced, dynamic environment.

Responsibilities

• Provide direct patient care, including administering medications, taking vital signs, and performing resident assessments.
• Develop and implement individualized care plans, collaborate with interdisciplinary teams, and prioritize resident-centered care
• Maintain accurate and detailed records of patient care, including medication administration, treatments, and test results
• Engage with residents, families, and caregivers to ensure effective communication and address concerns
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives, identifying areas for improvement and implementing changes to enhance resident care

As a Licensed Practical Nurse (LPN), you will have the opportunity to work on a flexible schedule, providing personalized care to our residents while promoting a positive and supportive environment.
