Greenbrier Senior Living's Timberdale Trace in Owatonna, MN is seeking a compassionate Licensed Practical Nurse (LPN) to deliver quality clinical care to our residents, following individualized care plans and state regulations. You will administer medications, complete assessments, document care, and support memory care and assisted living teams.

Qualifications

  • Current LPN license in Minnesota.
  • Current CPR certification.

Responsibilities

What You'll Do

As a Licensed Practical Nurse (LPN), you will play a vital role in providing quality clinical care and ensuring the health, safety, and well-being of our residents.

Responsibilities include:

  • Provide direct resident care while following individualized care plans.
  • Administer medications in accordance with state regulations, physician orders, and community policies.
  • Complete resident assessments as required by state regulations and community policy under the direction of the Director of Assisted Living.
  • Observe residents for changes in condition and promptly report concerns to the Director of Assisted Living and/or Director of Memory Care.
  • Accurately document resident care, observations, communications, treatments, and clinical procedures by the end of each shift.
  • Develop, update, and maintain individualized resident care plans.
  • Promote a safe, comfortable, and caring environment for all residents.
  • Assist with additional clinical duties and projects as assigned.
Supervisory Responsibilities

This position reports to the Director of Assisted Living and/or Director of Memory Care. The LPN is expected to work independently, exercise sound clinical judgment, and may assist with supervising, coaching, training, and evaluating other team members as needed.

Qualifications

The ideal candidate will have:

  • High school diploma or GED required.
  • Current Licensed Practical Nurse (LPN) license in the State of Minnesota.
  • Current CPR certification.
  • Minimum of one (1) year of related nursing experience preferred.
  • Strong communication, organizational, and clinical assessment skills.
  • A passion for providing exceptional care to seniors.
Physical Requirements

The position requires the ability to:

  • Sit, stand, walk, bend, stoop, kneel, crouch, climb stairs, reach, and perform repetitive hand movements throughout the workday.
  • Lift up to 25 pounds independently.
  • Assist with transferring, repositioning, pushing, pulling, and moving residents using proper techniques and equipment.
