Job summary

Chatham Hills Subacute Care Center is seeking a Licensed Practical Nurse (LPN) for the day shift. You will provide direct patient care in the SNF and Long Term Care units, administer medications, monitor vital signs, and collaborate with physicians and families to ensure optimal outcomes.

You will also supervise CNAs, educate residents and families, and participate in care planning. Competitive hourly pay up to $39/hour with benefits and perks.

• Current LPN license.
