Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking a Licensed Practical Nurse (LPN) for day shift. The role involves comprehensive nursing care for SNF and long-term care residents, including medication administration and vital signs monitoring.

The LPN will collaborate with physicians, NPs, and families, provide education, and supervise CNAs to ensure quality resident care. An active LPN license is required.

Qualifications

• Current LPN license required.
• Experience in SNF or long-term care settings is a plus.

Responsibilities
